IMM5669 form Education section - what If I dropped out high school and got GED certificate?

Basically, what should I put in "Name of Institution" and "From / To" dates?

High school was supposed to be total of 3 years, but I dropped out in the 2nd year.
I got my GED certificate the next year.

Should I list both my high school I dropped out and GED, or just GED?

What would be the name of the institution?
