IMM5645 to be filled by non-applicant spouse?

Dear All,
Recently, we are asked to submit the additional family information form (IMM5645). Me and my Mother are applicants but my father is not going to Canada. My question is that, does he still has to fill the IMM5645 form? because when they sent us the medical papers they asked for his medical as well?

Yes, his information has to be filled whether he's going with you or not.

I meant does he has to fill the form? like we will fill our forms separately? because he is not applying for the visa, its me and mom who are applied and not him.
