IMM5563E Form

I've been trying to download the IMM5563E form in order to request my CAIPS notes, but unfortunately every time I click on the link to download/view the document I keep getting an error about Adobe Reader. I've got Adobe Reader downloaded on all 3 computers in my house and on my iPad, and I'm getting the same error no matter which system I use.

I have also tried searching on Google for the IMM5563E form and still no luck.

If someone is able to download the form or already has it (it should be the latest one saying 2013 at the bottom), then could you please upload it and send me the link or possibly even e-mail it to me please?

Use internet explorer to download the form and it should work.

I just used IE and worked fine.

right click on the link, then select 'Save link as..' or 'Save target as..' (depends on your browser). Then save to your computer (desktop / C: drive etc.) and try to open from there.


Update: Even when I directly clicked on the link, I got the error message, but when saved to computer it worked.
