IMM5406E - Unable to Download

Hi All,

Long-time lurker, first-time poster. I've been asked to download and fill out IMM5406E however the link I was given in the letter is likely incorrect:

It doesn't open and I'm guessing by the 'cicintranet' address, I can't open this and it's likely an internal link. The first hit on Google:

Also doesn't work as it displays a 'Please wait' message and appears to be a broken PDF (And yes, I'm running the latest software etc, I'm a developer by profession)

Does anyone have a copy of the IMM5406E form? The only one I can find is dated in 2013 and I'm guessing it has changed.

With thanks to the posters below, the issue is that these files won't open under MacOS or Preview. You must download them and open them in Adobe Acrobat on a Windows machine.

It's not broken. You need to download it and open with Adobe reader instead of opening it directly in the browser.

Thanks but I tried that. Doesn't work with Preview on MacOS anyway.

Can you confirm that it works in say Windows or similar?

Yes it works fine. You need Adobe. Preview can't open it

For some reason, these PDF downloads from CIC site works only on Internet Explorer - Please try this.
