Hi Guys..

thanks a lot to everyone who takes time out of their busy schedule to answer question on this forum.

I am a Canadian citizen of a 17 year old son and my spouse is in Fiji and she 2 kids age 15 and 18.

we have a dilemma with this form IMM5406.

I have a few questions about this form.

My spouse has to fill out Section A.. but where it says spouse does she have to put name on that line.


Also, her son who is 18 yrs old has to fill out this form as well..
(1) On which line in Section A does he write his name...the only place I can see that happening would be Applicant but I am not sure.
Plus he would need to also state his mom and dads name as well.
(2) Section B does not apply to him
(3). Section C applies but he would have to state my sons name as well


Also,the form says to complete the names in your native language..Are they only asking for the names of the individuals only and do they want the full names in the native language.
