Hi there
my application is in process under the Express Entry category.
Just received a message to upload two copies of the IMM5406 (myself, the principle applicant and my spouse).

Would appreciate if you could clarify my doubt here.
I do understand that we both have to fill and sign our copies of the form and that in my copy, i write my name in the applicant column and my husband's name in the spouse section.

Question is, in his form, should he write:
His name in the applicant section and mine in the spouse
OR
My name in the applicant and his in the spouse section (i.e. exactly like my form)

Did you complete this form? I have the same question, infact I got this question after the birth of my child. Both of us are in the USA. Should we submit separate copies of this form? and should we have one for your newborn child? (I understand not)
