IMM5406 Form and IMM5669E Form

Dear All,

Today I received a message to my CIC account stating that they require few documents and need to upload them within 7 days.

1. 1XX6XX96XX - MY NAME:
2. Additional Family Info IMM5406: A separate and newly completed Additional Family Information form (IMM5406) bearing original signatures. http://www.cic.gc.ca/english/pdf/kits/forms/imm5669E.pdf

In here they are saying to fill out IMM5406 form and have given a link to IMM5669E Form as well.

Can someone let me know whether I should submit both forms..

I am single, and not sponsoring for any family members.

IMM5669E form requires to be signed by an Canadian government official and an authorized Interpreter. Should I go to Canadian embassy and get it done?
