IMM5406 ADDITIONAL FAMILY INFORMATION ...Please!!! help!!

I'm a little confused with this form IMM5406 please someone tell me: How many forms(IMM5406)Do i have to send? I'm sponsoring my wife to come to Canada and we don't have kids, so I'm sponsoring only one person. I have visited many forums but I'm still confused...Is it true I have to fill up one of this forms? ....It makes not sense for me since I live in Canada and the documents check list states that this requirement is only for Sponsored people or applicant...Please somebody help me! I got everything ready and I want to mail the application tomorrow and I don't want delays in my wife process for just this stupid form.

1. You don't "fillup any application forms" it is your wife. So she completes the IMM 5406 showing her family and your name as the spouse.

Thank You MPP!! Now I'm ready to mail everything I just want to get done with this TOO MUCH STRESS!!
