IMM5257e form filling up need help from experts

Dear Experts

Below name is just an example and its not my real name.


I have a question and I am sure you can help me out. My birth certificate name is John A.R Peter. I am from India, when i apply for passport, passport people wont allow me to use initial for my name so i have to notarized affidavit stating : A stand for Adam and R stand for Ron. So my passport name is John Adam Ron Peter. My father's name is Adam Ron Peter.

My passport name is John Adam Ron Peter
My birth certificate and all other documents my name is John A. R Peter
I have notarized affidavit stating that both name belong to same person and that is me.

My question is..... In IMM5257e form there is a part I have to answer and it says ... Have you ever used any other name (eg ..nickname, maiden name, alias etc)... should I say yes or no? Please solve this confusion for me. Thank you.

Notarization serves no purpose in this case. Notarization does not prove accuracy, so it is worthless.
