IMM1017B does not look correct

*Hello everyone, so i am a CES student, applying for a coop-work visa for my program.

*I am in Vancouver, BC, i have been here since August 2016.

*i went to take a medical check, then they gave me this form, saying this was the correc form for my IMM1017B when i asked. Yet, when i looked online, this does not look like IMM1017B at all:

- the layout is different
- the barcode is UMI instead of UF

Please tell me:
- If this is the correct form of IMM1017B which is needed for the COOP-work visa application.
