Imm008 - Question 2: alternate name

In Imm008 question 2b) of personal details (provide alternate names), my wife writes her maiden name in the family names section. Do we have to put Not Applicable in the given name section?

Sorry if this seems like a silly question but I'm really not sure when to use this N/A thing I've read so much about.

You say your 'wife' puts her maiden name in family names section (Q1, yes)? Why is that, has she not taken your surname on marriage? Note that for question 1 it says 'exactly as shown on your passport'.

So if she has taken your surname on marriage and her passport has been changed accordingly, she should put her married surname in Q1 and her 'maiden surname' in Q2b.

Of course she may have retained her maiden surname on marriage, just checking to see which it is really......

For Question 2, she puts her maiden name in the 'Family name(s)' section, correct? Then, for the 'Given name(s)' section, we leave it blank, correct? It's mainly a question of when to write "not applicable" versus when to leave it blank.

No, I don't believe that that is correct. I think she should write her given names in Q2 as well. So:

1. Family Name: SMITH Given names: ALPHA BRAVO

2. Family Name: JONES Given names: ALPHA BRAVO

That's what I did for my wife anyway and it seemed to work!
