Confused here.
We do not consent to releasing information to the destination province, or to sharing our info with perspective employers.

The guide says

Question:
Is the provincial release mandatory?
The employers release is optional.

When writing your 'name' in the space provided is it acceptable to put N/A? My spouse (who is a long ways away physically) did not enter her name here, and i am wondering if i am allowed to write it in. The form is already signed.

As per my knowledge you can not put N/A.You have to write name and put yes because you already mentioned where you intended to live (top of this form).
