IMM0008 and IMM5406 - Addition of dependent child

Received an ADR requesting IMM0008 and IMM5406 forms to be updated and submitted, but when I try to open these forms through the link provided in the ADR pdf, it is throwing an error. Tried multiple times and on multiple days but same error.

Can anyone provide me these forms or resolve this issue?

got the same error, i also need to add my additional child born 2 weeks ago. i tried using my office's laptop and it worked. looks like software version issue.
