IMM 5710 form – Application to Change Conditions, Extend My Stay, or Remain in Canada as a Worker

Hi everyone,

We have received the Acknowledgment of Receipt (AOR) and are planning to apply for an Open Work Permit to my spouse. My spouse has been on a visitor visa for nearly 1.5 years and previously held an open work permit, though she never worked for any employer.

I have a few clarifications regarding the IMM 5710 form – Application to Change Conditions, Extend My Stay, or Remain in Canada as a Worker.
Specifically, in Question 3, it asks:

“I am applying for one or more of the following:”

1. Apply for a work permit with the same employer
2. Apply for a work permit for the first time or with a new employer
3. Restore my status as a worker

Which option should I choose?

How to fill out the application form
When you fill out your work permit application form, enter the following details in the “Details of intended work in Canada” section:

• Box 1: What type of work permit are you applying for?
  • Select “Open Work Permit.”
• Box 4: Job title and Brief description of duties
  • Enter “SCLPC FC OWP” in the Job title box.
  • Enter “SCLPC FC applicant in Canada public policy” in the Brief description of duties box.
