Imm 5708 from date?

I got my visitor permit till december 28th which i applied on sept. 7th..

I am applying for extension for visitor i am filling imm5708 form can anyone in which there is question. Current status in canada which is visitor..
From and to date.. can anyone plz tell me from date i should write?

Should i write from november 25th when my permit came or should i write when i applied for extension that is 7th sept.?
