IMM-5669: Resubmission

Hello All,

I am a PA from India and my application sent on 23rd August has been returned because of missing address information in the IMM-5669 form.

We are resubmitting the whole application.

We had the following query when filling up IMM-5669 form:

d) been refused refugee status, or an immigrant or permanent resident visa (including a Certificat de sélection du Québec (CSQ) or application to the Provincial Nominee Program) or visitor or temporary resident visa, to Canada or any other country?

Do we answer this question as a YES or a NO ?

Since our initial application was returned and this is our second attempt.. we were wondering what should this be.
