Dear All seniros,

I am preparing upload files after ITA.
I see some posts mentioned IMM 5669 and IMM0008_4e form.
But there is no where in my checklist requested such form to upload.

May I know why? Ans what I should do??

Are they mandentory to upload the 2 forms? Will I got rejection if I didn't upload the 2 forms?

Thanks!!!!!

As far as I know, IMM 5669 often referred to as Schedule A will be requested by the case officer, if required.

IM0008_4E, commonly referred to as Schedule 4 is a form which a provincial nominee will have to fill and submit. People generally submit Schedule 4 along with eAPR while some wait for the request. If you do not submit it as part of eAPR, you will be requested for the same within one week of eAPR submission. You can submit this form as part of eAPR, if you would want to. If you decide to submit it as part of eAPR you can submit it as part of LOE as there is no specific document placeholder for Schedule 4.

Stanlee,

Thanks a lot for your definitely clear explanation.
Now I understand 5669 submission is base on request by officer, I can submit it if it is requried.
Schedule 4 is either can be submitted base on request after e-APR, or it can be submitted along with e-APR.

But i will not get rejection even if neither of the 2 files were submitted while submitting e-APR.

Thanks again!

Dear Stanlee,

Do you know why someone will be required to submit IMM5669?
