IMM 5645e - problem signing it and other issues

I am applying for a TRV (I already have a valid work permit, but came back to my country for a family emergency and now need a visitor's visa to return to Canada).

However I am having a few issues with the imm 5645e (Family Information form):

1. First the electronic signature thing is not working. I can fill up every other boxes in the PDF (like parents name, occupation, date etc) but the electronic signature bar is not working. I cannot type anything there. Are we supposed to print it and then sign it by hand and then upload it there?

2. The form also says: "Complete all names in English and in your native language". Unfortunately I am unable to type in Bangla (my native language) in the pdf. It only allows English. So again, should I print it, fill it by hand and then upload it?

3. In the form, it asks what type of application it is: 1) Visitor, 2) Worker, 3) Student, 4) Other. Even though I am applying for a visitor's visa to return to Canada, I already have a work permit and will return to Canada to work again. So should I select Visitor or Worker?

Please clarify the issues (especially #1 & #2) so I can apply.

Ans 1 - Yes, you have to print that form and use black pen and sign it and then again scan it and upload.

Ans 2 - For this too, you have to follow same as answer 1. you have to print that form and use black pen and fill info in Bangla and then again scan it and upload.

Ans 3 - Use Visitor as option
