IMM 5604 Declaration from Non-Accompanying Parent/Guardian for Minors

hi... somebody help me pls... im a single parent of 2 boys... one of them is minor..ive lost contact with their dad in july 2015...he blocked mine and kids number..and nowhere to be found.. do i still need to provide this document? i have an affidavit of single parent custody and ive already stated that fact...thanks ; )

It is better to try to track him to sign IMM5604 unless you have a court order of your chilren's custody, it would be enough.

hi, just checking this part of the documentation. i have a similar concern with my toddler's father abandonment. i am still married to him and although i know where he lives but we technically not in good terms..in the event that he is not cooperative in signing the form, will a DSWD certificate of single parenthood (RA 8972) suffice?

hi, it may help somehow bt i thnk it would be better to let him sign the imm5604 or agreement that you can bring your child with you ( travel consent). try to explain to him that it is for the best interest of your child..i hope he would agree about it..or if you can file a court order re:child full custody

Hi ...how did you handle the imm 5604 form. Please let me know. I'm in a similar situation.
 
Hi all. Please I would like to know at what stage is this form required. IRCC required it from me after almost 6 months after AOR. Does this announce the end of the procedure?

It was required to be included when you submitted the app.

No, it does not "announce the end".
