IMM 5533 - Including pages of passport which bear a Canadian entry stamp

If you are in Canada and/or have previous travelled to Canada, you must also include pages of any passport in your possession which bear an entry stamp made by a Canadian authority.

My wife has 3 pages from 2 passports that show previous entry stamps into Canada(2006, 2009, 2010 and 2016).

Was wondering, do I just include these photocopied pages loose together with other supporting documents? Or would I need to add any additional information to these pages explaining the duration of each trip(from when to when) and which cities were visited on that trip? Thanks

This request is stated underneath the passport section, meaning you would include the scans right alongside the main scan of her passport.

No additional trip information is required.

if one of cadana entry stamp ( as a visitor) were on an expired passport and thrown away? not able to submit
would it be regarded as incomplete document submission and return the whole applicatio?
