In a situation where the date of birth of half siblings are not known and the siblings are not willing to provide the information, can I just put down their names on the form without date of birth? Or should I just omit their names from the form?

Put down everything that you know and then just explain on a separate sheet.

You must put an asterisk for each unknown date field. And also provide the explanation like canuck_in_uk has suggested
