IMM 5406 Section A problem

IMM 5406 must be filled by both applicant and spouse. I don’t have any problem for filling my form (as a main applicant). But, I don’t know what I must write in the section A of my wife’s (as a spouse) form in the applicant information?
I think maybe I must write her name as an applicant and leave the spouse section empty?
Or
Write her name as a spouse and leave the applicant section empty?
Or
I must write my name as an applicant and her name as a spouse and fill the other form sections with her family information. But, in that case how CIC will found which information is about my family and which is about her?
I am really confused?

in this form your spouse disclosing her personnel information and she is main applicant her name will be in applicant and you are as spouse and her family members detail will release accordingly ......this is one and only form where your spouse is written as applicant.....remember she must sign the form ......
