IMM 5406 for new child

I am unclear on who needs to fill the 'Additional Family Information' form IMM 5406. The instructions in the form say that it must be filled by -
1. The applicant
2. Spouse
3. All children over 18 (not applicable to my case)

The document submission page in my CIC account only allows for uploading only one IMM 5406 document.
- Does my spouse also need to complete this form?
- If yes, I am assuming it needs to be attached to an email and sent to the VO?

A: In your case you and your spouse will need to fill one form each. So ther will be a total of two forms to be attached in the placeholder.
The document submission page in my CIC account only allows for uploading only one IMM 5406 document.
- Does my spouse also need to complete this form? A: No
- If yes, I am assuming it needs to be attached to an email and sent to the VO?
A:Yes all document placeholder have place for only one pdf file. Scan each filled form into a file and upload.
And No, nothing needs to be sent by email to the VO.
