Finally I am able to complete this form. I hope below information will help others :

Assuming You got a spouse :-
Main Applicant : XYZ
Spouse : abc
1. Both main applicant and spouse have to fill forms(Total 2 forms) . i.e.
Main Applicant
Name :- XYZ Relationship :- Applicant
Name :- abc Relationship :- Spouse

Spouse
Name :- abc Relationship :- Applicant
Name :- XYZ Relationship :- Spouse

2. If any section is not relevant (for example :- No kids) then put "Not applicable"

3. You have to write all names in English and your native language. Use pen for writing in native language. You can use Black or Blue pen to write that.

4. You can use sign and date using black/blue pen.
