IMM 5406 Does the Spouse Fill this form out too? "Mother" would be spouse's...

Hi, it says on this form that the "spouse" should complete their own copy of this form.

I'm Canadian (wife) and I am sponsoring my husband (American). Do I fill out his name for "applicant" my name obviously for "spouse" and my parents' names for "mother" and "father"? Since this form falls under the sponsored checklist I initially thought that my husband only needed to fill it out.

Can someone who has already been approved for their PR tell me what they sent in?

It is under Sponsored person and family members so i would think just the applicant .

Thanks for responding. That is what I thought but when I do a search for this question on the forum some people say that both spouse and applicant have to fill out the same form. It does say that "the sponsored person" plus each family member must fill one out. I might just fill it out to be on the safe side.

Lets hope someone with more experience can answer this , as i was under the impression that only the" applicant" had to fill this in as, the sponsor is already a Canadian resident and no other family members were coming as dependents .

I (as the sponsor) did NOT complete/submit one of these forms. I was simply listed on the applicant's form as 'spouse', as suggested above. Hence, the applicant's mother and father were listed appropriately, not mine.

Yes, only your husband need's to fill it - he is the only one being sponsored. If you were sponsoring your parents for example, then your mom and dad will complete their own copy of the form since they are spouses.

Sponsor's forms: IMM1344, IMM5409, IMM5476, IMM5481, IMM5491 and IMM5540.

Applicant(s) forms: IMM0008ENU, IMM0008DEPENU, IMM5406, IMM5476, IMM5490 and 3912(the country specific requirements), if applicable.
