Want to make sure, for the IMM 5406 additional family information form, does my wife (who i am sponsoring from costa rica) have to fill up only section A and section D? I am only sponsoring her not any of her other family, thanks.

ok, never mind, I know exactly what to do now... just have a simple questions for someone to answer, on this form (IMM5406) under section C for brothers and sisters, She has to fill this out as well right? even though none of her siblings are accompanying her?? 1 more question, when they ask for the "place of birth" what are they referring too? the hospital i am guessing? thanks!

Yes, she needs to complete the information regarding her family members even though they are not accompanying her.

You need to put the full address for place of birth, not just the hospital name.
 
'Place of birth' - you just need to put the city, province/state, and country for this. The address of the hospital is not necessary!

No, it's not necessary. Hey, if you know it it won't hurt to include it, but they really are just looking for the city and country here.
 
in secion A, does my wife need to fill in the Mother and Father info too?

This form really kicked me in the butt. I looked around here, and my wife (sponsor) phoned CIC to ask. They said only I (the applicant) have to fill out A, B, C, and D. She does not have to fill out anything, only the sponsorship application. I put my spouse in part A, so there's no need to sign where it says "I certify that I do not have a spouse...". I have no children, so I signed for part B, saying "I certify that I do not have any children...". I put my siblings names and info in part C, and signed at the bottom, saying all the above information is true...

That should be it, me hopes. :-[
