I am going to take a medical in Ottawa before sending in my outland PR application.
I have an appendix C form and the photographs.
I have read the notes, the documents all seem to state that the IMM 1017 form will be provided by the doctor, but the receptionist has asked me to (well, she insisted that I) bring a section A form ! The section A form is, as far as I can tell an IMM1017 form. So is the receptionist wrong, do all DMP's actually have this form available?
I believe that after the medical I get the second copy (so I assume it is a multipart form).
Once the medical is complete where does it get sent?
How do CIC make sure the forms "meet up"?
Id the FOSS Client ID the same as the ID on my visitor record?

Up-front medical exams and forms (IMM 1017 EFC and Appendix C)
Up-front medical exams are performed when applicants arrive at their immigration medical exam appointment with a completed Appendix C – Medical Instructions form.
The Medical Report: Section A (IMM 1017 EFC) is used for up-front medical exams.
DMPs should have a supply of IMM 1017 EFC forms on hand.
DMPs must never give a copy of the immigration medical examination documents to applicants or their representatives.
The only document that is returned to the applicant is the second copy of the Medical Report: Section A (IMM 1017 EFC).
