IMM 0008 - Dilemma

I am struggling with answering few questions of my IMM 0008 and would appreciate anybodies opinion.

Question 5 - has two fields, the first one is pretty straight forward - asking about my citizenship. What confuses me is the second part of this question asking to provide your country of residence. I am citizen of Slovakia living in United States for over 8 years. Throughout my stay in the US I have always been in status and currently hold H1-B visa. How do I answer this question? I am not a resident of US, I residing here for many years.

Question 15 - is related to the question above. This question is asking my residential address (if different from mailing address). Should I put down in this question my US address or leave it blank?

your country of citizenship : Slovakia (country of nationality)


your country of residence : USA (the country in which you reside (live), you are a resident of the united states and a citizen of Slovakia

residential address :write (N/A)( not applicable) if your mailing address is the same.

what professional1 said. I am Russian citizen studying in Canada, and this is what I am putting on my application just now. Some people give their address at work (i.e. as a grad student, I could direct mails into my departmental address), and in this case, your residential address would be different form the mailing address.

Just another advise - do not leave any place blank. Must fill up either with "not applicable" or "nil" wherever it is not applicable to you.
