IELTS for Dentists

Dear Dentists,

Please I need assistance regarding below queries:

• 
  I am a dentist from india and wana know if I have to do my IELTS with General or Academic Module?
  Can any 1 please send me final checklist of all the required documents?(Preferably those with successful submissions).
  Although the benchmark of assigned 500 Dentists is already completed, Still for next opening , can I apply under same occupation?

I would recommend the cic-website http://www.cic.gc.ca/english/immigrate/skilled/apply-how.asp
where you will find all the neccessary information, including the latest checklist. You need the IELTS General Module.
