IEC Form Error

In submitting my application through my MyCIC account, I accidentally submitted the wrong family history form, when I should have submitted form IMMI 5645.

Has this mistake ruined my chances?
What are the steps I can take to rectify this mistake?
Should I re-submit my application?

Something similar happened to me. I forgot to provide information about other family members on that form. Got my LOI approved tho.
IEC applications are being processed in NY. I emailed CIC in NY ( NewYork-im-enquiry (at) international.gc.ca ), attached the
correct form and apologized for the mistake.
You won't get a response from them, just make sure you include your UCI ( if applicable ), file/application number, full name and other details
confirming your identity in the email.

From what I've heard, all ( I'm certain about German, Austrian, Swiss, Australian ) IEC applications are being processed there.
You can also cc CIC in your email. THeir email is question (at) cic.gc.ca
