I just receive PGWP have problem on Visa!!!

Hello, everyone,
I just received PGWP and I'm going to apply for visa.
But I found some problems once I apply for new visa.
1.IMM.. form is shown "Outside Canada"
2. I was asked to provide "travel route" i.e. air ticket
P.S. my status: I am in Montreal, QC Canada
