How to upload my photo to PR application???

I got my photo taken at Walmart, followed all specifications to a T.

Am I understanding correctly that I'm supposed to *take a picture* of both sides of my photo and upload them to my application?
Can it just be a JPEG or should I convert them to PDF format?

Should I crop the pictures so that my photo is the only thing in view?

I also forgot to add this:

Application says that each person on the application needs to upload their photo. Does this mean my sponsor needs to as well?
