IMM 5645 is a file you attach when you apply your TRV. I suggest when you re-submit your new TRV just update the form with your daughter on it.
Do submit a Letter Of Explanation ( this like your cover letter or what you want to inform the Visa Officer) and mention that you were not able to declare your daughter on your previous applications.
I think it is more important for you to make up on the grounds stated on your previous rejection letters (example no Travel History, you should have travel to other countries... Not enough funds, save more money..etc). So that your next application will not be refused.
