How to requet a health claim records in alberta

I used to work as H1B in U.S. was also unemploymed in Canada for more than a year. There is high possibility that I get RQ. I want to start earlier to request all necessary documents just in case. Please let me know how I can request health claim records in Alberta. I only see the case for Ontario.

It's a super easy process and is all automated, I just received mine in the mail. Just call Alberta Health Services (google for the toll free number) and one of the options is to request a copy of your medical visits. Obviously you need to have your health care number but the process couldn't be easier. They will mail you a copy of your records in 2-3 weeks.

I requested a Statement of benefits paid 4 months ago. It costs $25+tax for a family and it does not contain any medical information. It only has the dates and the name of the doctor you visited.
This should be enough for CIC.

You will have to send a signed request with a check. Call them first to make sure you include everything. It took them 2 months to mail ours.




Statement of Benefits Paid
Alberta Health and Wellness provides a Statement of Benefits Paid to any Albertan who requests one. A Statement of Benefits Paid indicates fees paid to practitioners for the last complete benefit year plus the current benefit year to date. A benefit year is from July 1 to June 30. Hospital service charges do not appear on the Statement of Benefits Paid, but may be available directly from the hospital. Albertans can contact the Alberta Health and Wellness automated request system at 780-427-0845 within Edmonton to receive a copy of their current Statement of Benefits Paid. To call toll-free from elsewhere in Alberta, dial 310-0000, then 780-427-0845.
Statements can also be requested for up to seven benefit years for a fee. Requests for statements that cover more than the free period must be in writing and accompanied by a cheque or money order. Contact Alberta Health and Wellness at the telephone number above for the fee and mailing address.

I should have added that I didn't request mine for CIC, therefore the free statement was enough for my needs.
