My passport mentions "Joshua Bell" in the given name and blanks in the family name. However, in my study permit, they mentioned "Joshua Bell" in the family name section and the given name was left blank (not sure why). I need to know what I should mention in the family name and given name sections in my imm5710 form required for filing my PGWP. Thank you for your help.
