If you run a google check you'll see that this form is either available at the panel physician's clinic (http://www.cic.gc.ca/english/resources/tools/medic/exam/instructions.asp) or IRCC will send you this for
if you do not get an upfront medical - http://www.cic.gc.ca/english/information/medical/medexams-perm.asp.
To get an upfront medical exam it's simple, all you need is proper ID. Fo me I was asked to bring my passport a photocopy of the passport and a passport size picture.
You do not need any forms to get upfront medical.
Like I've said before, if the clinic you're going to insists on this them get written confirmation from them so that you can upload as the reason why you're not able to get an upfront medical or look for another clinic.
