I am filling out the form under my Spouse Personal Activities in the last 10 years, I am the main applicant and my husband (Seaman) is my dependent.
I am not sure on how to fill it out, I'll give example;
Activity type: Employment
Since what year: 2016-05-21
To: 2017-02-03
Please describe what this person doing during this period: Able bodied seaman
Please provide the name of company: Friendly Maritime Services Inc. (Philippine base)
Country where activity took place: I put Philippines (BUT I am not sure if it is right, because they travel for the whole time)
