how to fill aditional family information

hello guys i was filling out this form. I am the main applicant and wife is accopmanying me. when I am filling i have to provide to write my name in applicant name and spouse name is spouse colum, but my wen wife is filling this form what shud she write in applicant box and spouse box? please guide me
