Can someone help with the Consent for Access to Information Form IMM 5744E ?
I'm the sponsor, Canadian living in Canada. My husband, the applicant for PR, lives overseas.
We do not have a designated 3rd party representative for our file.
I want to file a request for GCMS notes, so I need to have my spouse sign the Consent for Access to Information
and scan it for me so I can attach it the request when I file it.
But I'm uncertain about which section of the Consent Form must be filled out and by who.
So, the applicant, my husband, must complete section A and sign it and scan it for me.
But what about Section B. and Section C. ?
-- Reading again now as I type, I think I have it backwards.
It looks like I fill out section A with my own name and signature -- the sponsor and the person making the request from Canada
my signature on section A.
-- Section B. My husband the applicant, living overseas, his information belongs in Sectionn B. and he must sign Section B.
and scan it back to me.
-- Section C. I have no idea if we put anything here.
-- Section D. This doesn't apply to us.
*** Anyone know what is actually the correct approach to this form? Thank you.
