how to apply GCMS notes for my wife and dependent kid

I am Dec-Applicant . I want to know how to apply GCMS notes for my wife and dependent kid(Both are in Canada)
I have the URL but I am not sure how to apply for dependents.
Do I have to submit two requests or one?
Any documents I have to submit ?
Please advise

When you request your own case files, you will get one file with the information for both as they are your *dependents*. CIC just creates one file for the both file. I ordered mine and my wife's case notes were also visible in it. That being said, there might be certain parts that are not included that will need authorization from your wife.

No. You have to send a request in Main and include a consent form signed by your wife.
