How to apply for the remaining period in PGWP?

Hi,

I got my PGWP for only 2 years instead of 3 years due to expiry of my passport. My PGWP has case id:56. Now I got my new passport. Can anyone please point out the documents needs to be submitted when applying for the PGWP extension either online or through paper?
