Your medical condition is not an issue.

Also let's pretend that my app is denied for whatever reason inland. Is it possible to turn around and apply Outland after the inland?

Your app isn't going to be refused, so don't factor that into a decision.

Outland: likely a PR by the end of this year.

Inland: able to work near the end of this year, possibly a PR by summer 2019.

I feel like the border agent might ask how you're related to your BIL and once it's revealed he's your BIL he will ask why your wife isn't with you. I also don't know the legalities of minor children crossing the border without a parent present in the vehicle, so I would look into that. As long as you can show ties to the US (bills in your name with the address on, proof you pay rent/mortgage/whatever, proof of return ticket is great) you *should* be okay, but sometimes the border agents are sassy.
So my wife called cbsa any they said that she just needs a letter saying that I know and ok with her moving the kids to Canada. would it be best if I cross at a different crossing? what if I get a 2 week stamp or something like that?
