It is observed that in most of the cases principal applicants are required alongwith the medical request letter to submit one or more of the following documents:
1. New Application Form for Permanent Residence IMM0008 from the principal applicant.
2. New Schedule A Background/ Declaration form for the principal applicant and each family member over the age of 18.
3. New Schedule 3 Federal Skilled Worker form the principal applicant.
4. New Additional Family Information Form IMM5406 for the principal applicant and each family member over the age of 18.
5. Right of Permanent Residence Fee (RPRF).
6. Original Police Certificates for the principal applicant and each family member over the age of 18.
7. Evidence of sufficient settlement funds for the principal applicant and his/ her dependents.
8. Work Experience Resume.
9. IELTS etc.
10. Others
so the question here is - that has anyone recieved the medical request without the requisition for one or more of the above documents, prior to recieving Passport Request ?
