How long is a "Statutory declaration of common law union" document valid?

Hi,

I am about to extend the work permit for myself and my common-law partner (I'm the primary applicant, and my partner is on an open permit). It is less than two years since we extended the last time. Can we re-use the same Statutory declaration of common law union [IMM5409] form? It is dated May 2010.

I tend to work on short contracts and so, I have to renew our work permits every year or so, and I'd rather avoid the hassle (and fees) to renew the IMM5409 document every time if it is not needed (it needs Commissioner of Oath signature). I looked in Guide 5553 (Applying to Change Conditions or Extend Your Stay in Canada - Worker") but couldn't find any info on this.

I called CIC, but I couldn't get a clear answer from them, only that in my case I needed to get a new form, and so I did.

I don't know where you are located, but if you are in Toronto, check out this guy. I've used him twice. He is quick and cheap. No hassles.
