How likely is refusal after everything is passed?

For those of us who passed eligibility, criminality and security but awaiting final decision, how likely is it that a final decision by supervisor overturns the recommendations from officers?

Is your eligibility recommended passed or passed? Recommended passed is given by the case analyst and passed is given by the senior officer. So if you have passed everything then the senior officer has approved your case.

A final decision has not been made yet, which I'm assuming means my file has yet to go to a senior officer for review. My GCMS notes say "Final Decision Pending" and I can see "CPA Eligibility Decision: Passed". I'm assuming CPA means Case Processing Analyst?

I'm wondering how likely it is a senior officer's final decision goes against this recommendation? :)
