How does upfront medical exam look like? question about my medical history

Hi,

I'm planning to do a medical exam soon for the new PR policy opening in May.
It's probably not a big deal but I have a tiny concern.
Like three years ago when I had gone through a medical exam for my study visa, I think I might have some of my medical histories that I forgot to tell the panel doctor.
It's a small operation that I did back in like 8 years ago.
My question is, would it be an issue if the medical history doesn't 100% match from the last time?
Should I not tell the doctor like I did last time or should I just tell even If I didn't tell?
Do 8 years ago medical history still count?
I revealed it when I applied for my insurance in Canada, but I doubt the government can check that?
Any advice will be appreciated.
