I need some guidance on how to fill the IMM 5604 form which is a declaration from non accompanying parent for Minors migrating to Canada. In my case, my wife is principal applicant but not the biological mother of my child. Does the non-accompanying parent have to state my wife's name on the form as the person that the child is migrating with since she is the principal applicant or should it be both my name and my wife's name or just my name only?
Also, should my wife's name be stated in the section that states "Fill out this section if this child is travelling to Canada with someone other than the parent/guardian listed above"?
I would appreciate some guidance please.
