How 6F table works

I am not sure how the 6F (Education/Experience) table works. Below is what I am inclined to include my stuff. Please correct me I am wrong. I first mentioned Work timeframe and then education timeframe as I have both education and experience at the same time.

From To Work/Education Employer or school attended City, Country
2012 2015 Work Xxxx Canada
2011 2012 Work Xxxx Canada
2015 2014 Education XXXX Canada
