asking Appendix A which the give me the Appendix A guide form

Question:

Please complete the following information for You and all your family members,whether accompanying or not.

We do not need the passport not passport numbers of non- accompanying family members. PLease write legibly.


I travelled alone and we dont have children either.

I am confuse of this form, should I stil put my husbands name?his in canada and his canadian citizen.
