Help with Quebec forms

I'm filling out the Quebec forms and I have some questions, hope someone can help me:

1) FORMULAIRE D'ENGAGEMENT (UNDERTAKING FORM – FAMILY CLASS) (A-0546-GF)
- Section 3: I didn't understand what I have to do.
I don't have any children and no one is coming with me.
Should I leave it blank?
At "Nom de famille et prénon" it's my name or the sponsor's name?

- Do I have to send TWO signed copies of this form or just the last page?

2) SPONSOR'S OR COSIGNATORY SPOUSE'S DECLARATION OF AUTHORIZATION (A-0527-A)
- My wife has never been married before.
Do we have to send this form all blank or just don't send it?

3) PAYMENT BY CREDIT CARD FORM (A-0591-A)
- Can I use my mother-in-law's (who lives in Montreal) credit card?
- Reason for applying is only the box "application for an undertaking - Family class"?
- "Date" means what? Today's date? The day for debit?

4) CSQ
- No questions about this one.

It's only this 4 forms? Am I missing something out there?

Hello!

1.
- section 3 - leave it blank. if you want peace of mind (and them to think you didn't leave it blank by accident), write N/A in one of those fields in the chart (or not applicable) - it's not mandatory though
- your 2nd question i'm not sure which section you mean from this form. if it's section 1 it's clearly your spouse's name (the sponsor), if it's section 2, it's the sponsored spouse's name / principal applicant's name ( so you)
- from what i remember i sent two COMPLETE copies of this form.

2. do not send this form at all, it doesn't concern you.

3.
- yes you can use any valid credit card, just put your mother-in-law's details on the form (first section: credit card holder's info - last section with "principal applicant" is to be completed with your info, so the sponsored spouse)
- reason for applying - application for undertaking, family class - correct
- date: it's today's date, when you decide to send everything, put same "today's" date on all forms

that is all, in total you have 3 forms: the undertaking application form (for the spouse) , application for CSQ (completed by the sponsored person) and the payment by credit card form.
DO NOT forget to send documents with it (citizenship card or birth certificate etc, what they require)

Family members concern spouse and children or step-children.
Also you must include a utility bill with with address on it like Hydro Quebec or internet/cable ...

Thank you, but he said 4 forms, and the link that you sent me is not the form... And for the Checklist to send it to CIC when I applied for PR, there are multiple boxes to tick in - so it is a kind of form to work with... So I just wonder if there is any Checklist like that?

Thank you! I actually checked that link and worked on it, but when I saw DanSlh mentioned 4 forms which included Checklist form, I just wanted to make sure if I miss any form or not.

You should be able to fill it online or once saved as PDF. Make sure you are filling it in french and not in english!!
They also ask 2 copies of that form, so make sure to send 2 of them!

Download the pdfs and edit them on your computer. You need to send two of one form, only French ones are valid. Just use the English ones as a guide. It'll take about 3 weeks for them to get back to you. If you're Outland, then you'll have a nice CSq that you can't use for anything until you land using your pr.

At the starting of the forms they explain what and why you need to fill the form. The english forms are only given to help understand the questions written in french ( you won't be able to fill the ones in english)
And yes, 2 copies of that form ( they will send you one back once you get approved for CSQ with their decision on the last page)
